Please submit Patient’s Information below.

Instructions For Patients:

  • Please bring a referral slip, any X-rays, and a photo ID with you to your appointment.

  • Minors must be accompanied by a parent or legal guardian.

  • Please bring a list of all medications and doses you are currently taking.

  • There is an hourly parking garage in our building.

  • Street parking is also available.

Patients Anticipating Intravenous Sedation or General Anesthesia:

  • It is important that you do not eat or drink anything 6 hours prior to surgery.

  • Please wear comfortable, short sleeve or loose fitting clothing.

  • An escort/driver must stay in the facility during your entire appointment.
